Veterans Case Manager

The Veterans Case Manager position is a Full Time benefit eligible position, Monday Friday 12:00 pm - 8:30pm, occasional nights and weekends at our Van Keuren office located at 2223

E. Genesee St. with travel throughout the assigned portfolio. Salary range is $25-$28/hr.

The Veterans Case Manager assists Housing Visions Veteran clients residing in a permanent supportive housing setting by assessing their needs, developing service plans, and collaborating with appropriate providers to work together towards healthy, independent living. They will provide individual case management, advocacy, and act as a referral source to other community and mainstream resources. The overall goal is to support new or existing clients in securing and maintaining safe, quality, affordable housing and fostering a supportive environment which reflects the mission, vision, and values of Housing Visions.
