Physical Therapist – Skilled Nursing Facility

Flagstar Rehab is seeking Physical Therapists (PT) for skilled nursing facilities. Flagstar Rehab puts PTs in the driver’s seat when it comes to managing their careers and work-life balance. Therapists have the flexibility to choose assignments that fit their schedules, with the added benefit of a “choose the way you earn” model and support in setting up a corporation if they prefer independent work.

With daily updates and clear communication about facility needs, therapists always know their options, and remote onboarding ensures a smooth transition into new roles. This flexibility allows PTs to be present for both their families and the patients they serve, while filling critical gaps in geriatric care.

• W2: $44 per hour
• 1099: $55 per hour

• Current NYS PT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice / Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating conditions.
• Monitor and document patient progress with the use of innovative EMR software.
