In this position, the Social Worker provides comprehensive psycho-social assessment, management and support to at risk patients and families at the Upstate Pediatric and Adolescent Center. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. In addition, the Social Worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual's health needs.
Additionally, the Social Worker will participate in quality initiatives within the assigned areas. Candidate must possess communication skills, have knowledge of Social Determinates of Health and available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.
L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience required.
* New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.
Three years relevant post-master's experience in a hospital and/or healthcare environment.
Work Days:Monday-Friday, days with occasional on-call/holiday coverage on rotation based on operational needs
