Core Responsibilities:

Conduct primary care rounds, diagnosing and managing chronic and acute conditions within the skilled nursing facility. Develop and implement effective, evidence-based care plans. Collaborate with interdisciplinary teams, facility leaders, and the post-acute care community to enhance patient care and coordination. Provide guidance and mentorship to Nurse Practitioners. Requirements:

Experience:

Minimum of 1 year of experience in Primary Care. Experience in post-acute care is advantageous. Background:
Candidates from Internal Medicine, Family Medicine, Hospitalist, or Emergency Medicine backgrounds are welcome. Licensing:
