What You Will Do
  • Build and maintain collaborative relationships with professionals, agencies, and community resources to support individuals.
  • Conduct comprehensive assessments and plan individual outcomes in partnership with families, carers, and other professionals.
  • Develop and oversee care plans, delegating tasks and managing caseloads efficiently.
  • Review and monitor care plans regularly, contributing to service improvements.
  • Maintain accurate, up-to-date records in line with organisational standards and statutory requirements.
  • Participate in team meetings and contribute to service development, ensuring equal opportunities are promoted.
  • Uphold health and safety standards while maintaining confidentiality of service users.
  • Provide supervision and guidance to students, newly qualified staff, and team members.
  • Manage complex cases and undertake projects to enhance case management and service delivery.
