Service Lead for the Integrated Hub and Hospice at Home
Beaumond House Hospice Care delivers personalised, high-quality end of life care across hospice, home, and community settings. Patients and families remain at the centre of everything we do. All staff are expected to uphold dignity, safety, confidentiality, and the highest standards of physical, psychological, and spiritual care.
This role sits within the End-of-Life Care Together Alliance and operates across organisational boundaries to deliver safe, coordinated, and person-centred care for people in North Nottinghamshire.
This role is based at The Integrated Hub in John Eastwood Hospice, Mansfield.

Main duties of the jobYou provide senior operational and clinical leadership for the End-of-Life Care Together Hub. You hold delegated responsibility for service delivery, workforce management, quality, performance, and use of resources on behalf of Beaumond House Hospice and the Alliance.
You lead integrated teams to support patient choice, enable people to remain in or return to their preferred place of care, reduce avoidable admissions, and improve discharge flow for people who are rapidly deteriorating. You work collaboratively with system partners to align services to agreed pathways, strengthen integrated working through neighbourhood teams, and deliver continuous improvement in the quality and safety of end-of-lifecare.

About usBeaumond House Hospice supports patients and their loved ones to live as fully as possible across North Nottinghamshire. Ensuring our communities receive the care and comfort they need, free of charge, when every moment matters. Our Care is provided in people's homes across our communities and at the Hospice in Newark.
We treat every person with respect. We deliver compassionate, professional care that supports physical, psychological, and spiritual needs. We look for staff and volunteers who protect dignity, safety, and confidentiality.
Beaumond House Hospice Care is a registered charity. We provide four In-patient beds, Day Therapy, and Hospice at Home Care. Hospice at Home supports people with palliative and end of life care needs, often in the last weeks and months of life. Our nurses and health care assistants provide assessment, personal care, emotional support, advice, and advance care planning in the persons preferred place of care.
We are contactable 24 hours a day, 7 days per week, to provide a friendly ear, general information or to signpost patients, relatives and carers to appropriate resources and services.

Leadership and Accountability
Provide visible senior leadership across the Integrated Hub and Hospice at Home service
Line manage Hub and Hospice at Home staff, setting clear objectives aligned to Alliance and hospice priorities
Maintain clear accountability, governance, and escalation routes
Identify, manage, and upscale operational and clinical risks appropriately
Promote a positive, inclusive culture that supports staff engagement, accountability, and improvement
Clinical Leadership, Quality and Governance
- Provide senior clinical leadership and oversight
- Ensure clinical supervision, professional standards, and safe practice are embedded
- Lead and support responses to incidents, complaints, and safeguarding concerns
- Contribute to the development and review of clinical policies and procedures
- Maintain professional registration and meet revalidation requirements
Service Delivery, Quality, and Improvement
- Lead delivery of the End-of-Life Care Together Hub model in line with Beaumond House strategic goals
- Accountable for quality, safety, outcomes, and performance across Hospice at Home
- Manage services against agreed performance measures, contractual requirements, and patient outcomes
- Drive admissions avoidance, patient flow, and timely discharge
- Redesign pathways and ways of working to meet demand and improve quality
