• Facilitate timely hospital discharges by coordinating multi‑agency collaborations to ensure safe and effective outcomes.
• Apply your knowledge of statutory social work, the Mental Capacity Act, and Best Interest decision‑making to safeguard vulnerable individuals.
• Engage with adults, carers, and other professionals to optimise care solutions within a demanding fast‑paced hospital setting.
• Tackle safeguarding responsibilities under Section 42 with confidence and expertise.
