We have a Relief Part Time opportunity for a Clinical Pharmacist to join our team at Jim Pattison Outpatient Care & Surgery Centre located in Surrey, BC. This position is available until approximately March 31, 2026 or until the return of the incumbent.

As a Clinical Pharmacist you will work to provide the following services to our patients:
- Individualized pharmacotherapy
- Counselling patients about their medication therapy
- Collaborate with physicians and other health care providers to develop, implement and monitor therapeutic plans
- Assist in the development of drug therapy guidelines for individual disease conditions
- Prepare and submit for publication in professional journals and case studies
- Act as department resource person within your assigned area
- Participate in medication use evaluations, quality improvement initiatives and clinical drug trials
- Provide mentorship and supervision to pharmacy students, residents and new staff
- Ensure prescriptions are validated and dispensed accurately; address medication safety concerns with the health care team; document professional practice activities

Detailed Overview
The Clinical Pharmacist provides clinical pharmacy services including individualization of patient medication therapy, patient medication counselling, and consultation with physicians, nurses and other health care providers to discuss therapeutic plans. Assists in the development of drug therapy guidelines for individual disease conditions and supports the Pharmacy & Therapeutics Committee by preparing summaries and recommendations on drug usage. Participates in medication use evaluations and clinical drug trials.
Provides instruction and supervision to pharmacy students and residents. Also performs drug distribution duties including processing prescriptions, validating prescriptions, and ensuring safe and accurate dispensing.
Responsibilities
- Consult with physicians, nurses and other health care providers to develop therapeutic plans and ensure documentation in the patient health record.
- Individualize patient medication therapy based on chart reviews, lab results, and clinical assessment; modify plans as needed.
- Provide patient medication counselling and education to patients and families.
- Assist in developing drug therapy guidelines and monitor effectiveness of guidelines on patient outcomes.
