We are seeking a full-time Nurse Practitioner to provide compassionate hospice, palliative, and house call care within the community.
Responsibilities- Collaborate with a multidisciplinary team to deliver comprehensive care for patients with advanced or terminal conditions.
- Conduct home visits in private residences or care facilities, focusing on care planning and symptom management to support aging in place.
- Ensure continuity of care by performing face-to-face visits to meet regulatory requirements and address patient needs.

Valid licenses in Idaho and/or Oregon, a valid Driver's License, and board certification in Hospice & Palliative Medicine or Primary Care.
