* Job Description:
** - Are you passionate about restoring function and enhancing the quality of life for your patients? - Join a dynamic team dedicated to helping individuals achieve their maximum performance and independence!

* Key Responsibilities:
** - Evaluate and treat patients with medically prescribed physical therapy programs.
- Supervise Physical Therapy Assistants and lead initial admission/evaluation visits.
- Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans efficiently.
- Communicate OASIS scores to the Clinical Team Manager within established timelines.
- Perform other duties as assigned that align with your skills and training.
** Territory Details:** - Covering Milpitas, Saratoga, Monte Sereno, Los Gatos, Campbell, San Jose, Morgan Hill, San Martin, Gilroy, Coyote Point, Sunnyvale, and neighboring areas.
** Shift Information:** - Days: 8am-5:30pm (Flexible schedule based on patient's availability) - Weekends:
One weekend day per month
** Requirements:** - Minimum of one year of Home Health experience.
- Active BLS/BCLS certification.
