The posted compensation range of $143,000 - $187,300 (annual salary) + Management Incentive Plan is a reasonable estimate that Pathways Home Health & Hospice believes might be paid for this job based on the circumstances at the time of posting. Responsibilities
Director, Clinical Services (DCS) is responsible for the planning, direction, coordination, supervision, evaluation and provision of all nursing services at the assigned facility location.Lead the education, training, and development of employees and oversee the coordination, continuity, and quality of clinical care, as well as the management of patient services.Participate in the recruitment of employees and provide supervision to facilitate quality care and patient satisfaction; monitor, evaluate, and take action on clinical measures of performance.Adhere to the Compliance Program and maintain confidentiality of HIPAA-protected information; ensure confidentiality of client information.Function as an effective team member to achieve location and organizational goals and objectives; maintain professional availability to assist staff.Maintain effective communication and confidentiality of information; supervise and monitor the care provided to patients and ensure safe, therapeutically effective care; conduct on-site nursing supervisory visits.Direct and oversee clinical services to ensure quality patient care in compliance with regulatory, operational, and financial requirements.Ensure patient plans of care are individualized and appropriate; ensure accuracy and timeliness of clinical records and compliance with all regulatory standards.Monitor clinical outcome results and develop/implement improvement plans using SHP and company data to drive positive outcomes.Monitor and evaluate clinical performance measures (productivity, documentation, patient care) and follow through with action plans based on results.Oversee ongoing development of clinical staff knowledge, including OASIS, physician orders, and other clinical documentation tools.Provide timely communication of regulatory, operational, and financial information to clinical and applicable office staff; lead education initiatives (new hire orientation, in-services, meetings, case manager meetings, formal clinical education).Complete assigned workflows accurately and on time; participate in branch activities such as strategic planning, budget development, and financial performance; provide sales support when necessary.Perform patient visits, supervisory visits, and shared visits as needed or directed; ensure appropriate on-call coverage and participate in rotation as needed.Participate in office functions including intake, payer verification, payroll, billing resolution, and laptop support as directed; act as back-up to Market Director, Operations.May serve as Administrator of Record with the State Regulatory agency.Quality
Assure appropriate service utilization and correct OASIS scoring; validate visit frequency based on assessment scoring and ensure field staff perform visits appropriately; arrange appropriate preceptors when needed.Review all occurrence and complaint reports and perform related follow-up.Coordinate QAPI program activities, including data collection/analysis, committee oversight, improvement initiatives, education, evaluation of effectiveness, and communication of results.All other duties as assigned.Qualifications
- Current Registered Nurse (RN) license to practice in CA is required.
