Key Responsibilities- Deliver targeted 1:1 and group support to children and families
- Complete assessments and develop outcome-focused support plans
- Work alongside Social Workers, Schools, Health, and Early Help Teams
- Support families with routines, boundaries, housing, budgeting, parenting, and accessing community resources
- Keep accurate records and contribute to multi-agency meetings
- Promote positive change, resilience, and independence
