MDS Coordinator (RN or LPN) – The Architect of Care
Pay: Starting at $43/hr (RN) or $40/hr (LPN)
Schedule: Full-Time / Day Shift

Are you a detail-oriented nurse who loves the "puzzle" of clinical documentation and care planning? As our MDS Coordinator
, you are the vital link between resident care and facility excellence. You ensure that every resident has a roadmap to wellness (Care Plans) and that the facility is accurately recognized for the high-level care we provide through the Minimum Data Set (MDS).

Key Responsibilities
- MDS Management: Coordinate, record, and monitor all MDS assessments in strict accordance with CMS guidelines and PPS rules.
- Care Planning: Lead the development of interdisciplinary, written plans of care with measurable objectives for every resident.
- Financial Stewardship: Understand and apply concepts of RUG IV and state case mix to ensure accurate reimbursement and avoid payment penalties.
- Family & Team Liaison: Coordinate patient/family care conferences and act as the primary communicator between medical staff, families, and payor sources.
- Quality Oversight: Monitor quality metrics and reporting programs to identify trends and implement improvement actions.
- Staff
Education:
Empower your team by providing education on supportive documentation and late-loss ADLs.
- Education: Graduate of an accredited RN or LPN program.
- Licensure: Active Arizona State RN or LPN license.
- Certification: CPR certification (required upon hire).
- Experience: 6 months in a long-term care environment preferred.
- Skills: Expert knowledge of CMS guidelines, excellent multitasker, and a "person-centered" approach to care documentation.
