Position Summary

This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and the standards of professional nursing practice. It is accountable for the quality of nursing services delivered by self or others under their direction, and utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide, and evaluate care appropriate to the physical and developmental age of assigned patients.

Functions
  • Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs; reviews patient history; interprets diagnostics and laboratory data; reports abnormal results to the physician(s) and other appropriate caregivers; completes assessments and reassessments according to patient need.
  • Formulates and revises a plan of care, including the discharge plan, based on assessment data and patient, family and health team input; ensures goals are realistic, measurable and developed with the patient and family.
  • Implements the plan of care through direct patient care, coordination, delegation and supervision of health care team activities; provides care based on physician orders and nursing plans in compliance with policies, procedures and regulatory requirements; delegates appropriately and provides nursing supervision.
  • Evaluates the patient’s and family’s response to care and teaching; revises the plan of care as needed; evaluates patient progress towards goals and expected outcomes in collaboration with other health care team members.
  • Documents assessment, planning, implementation and evaluation in the patient record; ensures documentation is legible, timely and consistent with policy.
  • Provides care based on best evidence; may participate in research activities; participates in unit or facility leadership; mentors and precepts when appropriate.
  • Contributes to society through activities that lead to excellent patient outcomes; participates in professional organizations; may supervise staff and workflow in some roles.
  • Interacts with all levels of staff in various departments, physicians, patients, families and external contacts; provides spiritual support when requested; synthesizes data from multiple sources to support patient and family response to changes in health status.
Minimum Qualifications
  • Current, valid RN license in the state of practice or temporary/interim/compact RN license.
  • BLS certification required; additional certification or continuing education may be required based on area of practice.
  • Minimum of one year experience in an acute care hospital setting or acute care setting within the past 12 months as an RN in the specialty area.
  • For certain roles (e.g., Home Health) a valid driver's license, active auto insurance, and appropriate fingerprint clearance cards as required.
