Overview
We are seeking Registered Nurse to service our patients throughout the Pee Dee area. The Nurse Case Manager role is a critical part of the patient’s care team. The nurse will visit patients, facilitate with the appropriate provider, and travel to patients’ homes and facilities to evaluate patients and initiate telemedicine visits. Provide quality healthcare in adherence to all applicable laws, regulations, and policies within the scope of practice.

Sumter, Florence and surrounding areas.
- Visit patients in their homes or facilities (ALF and ILF) within the designated service area and coordinate with providers to initiate care.
- Facilitate receiving all medical records from the patient’s primary provider and specialists.
- Review medical records and complete required consents with patients.
- Enroll patients in Care Management if eligible and initiate a Care Management Plan of Care if the patient is eligible.
- Capture diagnoses at the highest specificity, identify gaps, and ensure they are accepted.
- Complete AWVs and cognitive impairment screenings as part of the evaluation process.
- Complete Social Determinants of Health (SDoH) assessments and/or screenings, and Advance Care Planning (ACP) documentation.
- Evaluate for home health, hospice, palliative care, or consults with Your Health Specialty Division, etc.
- Evaluate for RPM devices, resources, or tools to improve quality of life.
- Coordinate and communicate with the patient’s health care team, including physicians, therapists, home health or hospice providers, and other care plan participants.
- Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
- Provide post-discharge education and assess for adaptive equipment and durable medical equipment (DME).
- Assess for a safe home environment and evaluate acute conditions or follow-up from previous visits.
- Document and log Care Management activities and keep care plans up to date in collaboration with the care team.
- Facilitate telehealth visits with providers when necessary and coordinate with hospice interdisciplinary teams.
- Participate in IDG meetings, when necessary.
- Must be a Registered Nurse with a license in good standing with the appropriate board/issuer.
- Minimum of three (3) years of clinical experience preferred.
- Experience in community settings preferred.
- Proven ability to effectively communicate and collaborate with interdisciplinary care teams, patients, and caregivers.
- Strong written and verbal communication skills.
- Basic computer knowledge and ability to use relevant health IT tools.
- Ability to manage and demonstrate effective leadership and teamwork; able to manage multiple responsibilities and exercise good judgment.
- Flexible hours and ability to travel between offices and facilities; must be a licensed driver with a valid automobile insurance and in good working order.
