Job Title: Travel RN – Inpatient Care Manager
Location: Summit, NJ

Contract Dates: April 6, 2026 – May 30, 2026
Schedule: Monday – Friday | 8:00 AM – 4:00 PM | 37.5 hours/week
On-Call/Weekends: None
- Develop and manage individualized case management plans by reviewing clinical findings, diagnostic reports, and patient assessments.
- Collaborate with physicians and interdisciplinary teams to coordinate patient care progression and discharge planning.
- Identify high-risk patients who may require psychosocial support and coordinate referrals to Social Services.
- Implement interventions aimed at preventing hospital readmissions.
- Develop and maintain transition of care and discharge plans with the healthcare team.
- Monitor length of stay (LOS) and participate in care rounds to address discharge barriers.
- Coordinate communication between patients, families, physicians, and care teams to ensure smooth transitions across levels of care.
- Identify and escalate financial or payer-related issues that may affect the patient’s discharge plan.
- Maintain knowledge of CMS, Medicare, and Joint Commission regulations, including Observation status and the 2-Midnight Rule.
- Educate patients and families regarding care progression, discharge planning, and resource utilization.
- Report potential risk management or quality concerns through the appropriate channels.
- Active Registered Nurse (RN) license
- Strong communication, negotiation, and problem-solving skills
- Knowledge of case management standards and healthcare regulations
- Prior clinical experience managing hospitalized patients
- Strong computer and documentation skills
- Previous acute care case management experience
- Case Management certification or formal training through a professional organization
- Estimated Gross Weekly Pay: $2,545
- Weekly Tax-Free Stipend: $1,533
- Estimated Hourly (W2): $27.00/hr
