Our client is seeking a Registered Nurse in Seguin, TX, and surrounding areas! If you have experience in a home health setting this opportunity could be the perfect fit for you!

We seek a compassionate and skilled Full-time Registered Nurse to join our Home Health team in Sugar Land, Texas. As a Home Health Registered Nurse, you will play a crucial role in providing high-quality care to patients in the comfort of their own homes, promoting their health, independence, and quality of life.

  • Conduct comprehensive patient assessments and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication administration, and IV therapy
  • Educate patients and their families on disease processes, medications, and self-care techniques
  • Collaborate with physicians, therapists, and other healthcare professionals to ensure optimal patient outcomes
  • Accurately document patient care using electronic health record (EHR) systems
  • Monitor and evaluate patient progress, adjusting care plans as needed
  • Adhere to all home health regulations, standards, and safety protocols
  • Participate in quality improvement initiatives and team meetings
  • Manage time effectively to complete all required visits and documentation within specified time frames
  • Bachelor of Science in Nursing (BSN) required
  • Current Registered Nurse (RN) license in the state of practice
  • Minimum of 1-2 years of experience in home health nursing or related field
  • Strong clinical skills and ability to provide comprehensive patient care
  • Excellent communication and interpersonal skills
  • Proficiency in patient assessment, care planning, and documentation
  • Experience with electronic health record (EHR) systems
  • Current Basic Life Support (BLS) certification
  • Home Health Nurse certification preferred
  • Knowledge of home health regulations and standards
  • Understanding of Medicare and Medicaid guidelines for home health services
  • Ability to work independently and manage time effectively
  • Reliable transportation and valid driver's license
  • Flexibility to work varying schedules, including some evenings and weekends as needed
