. Called to serve patients in their homes, the Registered Nurse provides personalized, one-on-one care to help patients regain independence and achieve health goals. You’ll set the standard for the Vital Caring experience, delivering skilled, compassionate care and building meaningful patient relationships. This position offers a flexible schedule and an on-call requirement:
Monday - Friday 5p-8a, Saturday and Sunday 24 hours.

• Evaluate and assess patients to determine appropriateness for care.
• Develop patient-specific care plans using available tools and resources, with appropriate level and frequency of care.
• Communicate with the care team, physicians, payors, patients, and families to address care needs and fulfill the patient’s care plan.
• Deliver high-quality skilled care and supervise the care of other care team members.
• Thoroughly document care delivery daily in our EMR system.
• Contribute to a culture of caring through individual accountability and teamwork.

• Current RN license, valid state driver's license, and reliable transportation.
• One year of RN experience in an acute or clinical care setting is preferred; home health experience is preferred.
• Compassionate in care delivery, focused on results; solution-driven, self-motivated, and responds with urgency.
• Love learning, motivating, and inspiring patients to reach their goals.
• Enthusiastic about practicing at the highest level of the RN license; comfortable with technology; HCHB experience is a plus.
