About YAD HealthcareYAD Healthcare proudly operates several skilled nursing and long‑term care (SNF/LTC) facilities across Virginia. We’re dedicated to providing personalized care and quality rehabilitation for both short‑term and long‑term residents.
We’re seeking a compassionate and skilled Respiratory Nurse (RN or LPN) to join our team. In this role, you’ll travel between 2 of our Virginia facilities to support residents with pulmonary (lung) needs by carrying out care plans developed by our Certified Respiratory Therapist.
This is a day‑shift position on a rotating schedule
, ideal for nurses with at least one year of clinical experience who are passionate about hands‑on patient care and teamwork.
- Work closely with a dedicated healthcare team to support patients with respiratory conditions.
- Contribute expertise as a member of a multi‑disciplinary team to provide comprehensive treatment strategies and positive outcomes for residents and the pulmonary health program.
- Communicate effectively with rehab team members about patient findings, concerns, or program improvement opportunities.
- Strive toward proficiency in clinical competencies and best practices.
- Assess patients’ modifiable risk factors and provide guidance to support successful therapeutic lifestyle changes (TLC).
- Participate in educational efforts, classes, or individual sessions for patients.
- Assist in developing individualized treatment plans with other staff members.
- Monitor and document oximetry, heart rate, and blood pressure.
- Assess residents before, during, and after breathing exercises, making referrals or recommendations to patients, staff, or the Medical Director as needed.
- Maintain accurate initial and daily assessment records in patient charts.
- Immediately report any significant or changing patient conditions (oxygen desaturation, dysrhythmias, abnormal blood pressure, or status changes) to the supervising physician.
- Participate in the collection, analysis, and reporting of patient and program outcomes.
- Participate in emergency drills and mock codes regularly.
- Attend all scheduled staff meetings.
- Educate patients and families on breathing exercises and healthy lifestyle habits.
- Respond quickly to any changes in patient condition and communicate with the supervising physician.
- Active Virginia RN or LPN license (in good standing)
- Current CPR and ACLS certification
- At least 1 year of clinical experience
- Reliable transportation to travel between sites
- Ability to lift or move up to 50 lbs
- Must pass a background check and pre‑employment drug screening
