Position: Healthcare Access Coordinator - HCO2025
GENERAL STATEMENT OF DUTIES AND RESPONSIBILITIES
Possess in-depth knowledge of the Mass Health application and enrollment processes, the Consolidated Appropriations Act (CAA), and the 1115 Waiver, and actively support department leadership in planning and implementing new 1115 Waiver requirements to ensure compliance and effective service delivery. Must collaborate with internal divisions, other correctional facilities across the Commonwealth, and partner agencies to ensure coordinated and effective implementation of all Mass Health requirements.
Apply expertise in telehealth platforms to plan, implement, and manage telehealth and telephonic services across the Suffolk County House of Correction and Suffolk County Jail.
DUTIES AND RESPONSIBILITIES
- Competent in the Mass Health application and enrollment processes as well as the Consolidated Appropriations Act (CAA) and 1115 Waiver.
- Must be able to attain and retain Mass Health Certified Application Counselor Certification.
- Able to work in two county correctional facilities and collaborate with all internal divisions to prepare incarcerated individuals for reentry.
- Competent in telehealth processes, platforms, and medical/mental health discharge planning needs.
- Must be able to communicate verbally and in writing with internal and external partners.
- Ability to assist in the planning and implementation of the new 1115 Waiver requirements with Department leadership.
- Track incarcerated individuals under the CAA mandate and collaborate with internal divisions and partners to ensure compliance.
- Plan, implement, and oversee the scheduling of telehealth and telephonic program intakes/screens, social service connections, virtual pre-release appointments.
- Assist in hiring and overseeing support staff.
- Ability to collaborate and work with other county correctional facilities throughout the Commonwealth and other agencies.
QUALIFICATIONS
A Bachelor's Degree in Health Administration, Public Health, Nursing, Social Work, or a related field is common, or an Associate's Degree or equivalent experience or a combination of education, experience, and training.Prior experience in a healthcare setting, such as a care assistant, medical discharge planning/case management, or in healthcare program management, is highly valued.Proficiency in Microsoft Office Suite, electronic health records (EHR) systems, and other relevant healthcare software is necessary.Working knowledge of virtual meeting platforms such as Zoom, Web Ex, Doxy.me, Google meets, Microsoft teams, and other HIPPA compliant platforms.Critical thinking and strong problem-solving skills are needed to address diverse and complex challenges in the healthcare system.Familiarity with healthcare policies, regulations, and Mass Health processes.A strong understanding of patient privacy laws and confidentiality is required.Ability to perform duties with a high degree of discretion and confidentiality.Ability to communicate effectively in oral and written form.Valid and renewable driver’s license is required.Ability to work accurately and independently in a fast-paced environment.Attention to detail, creativity, flexibility, and organizational skills are crucial.ESSENTIAL FUNCTIONS
Maintain working knowledge of Mass Health, Mass Health application process, CAA, and 1115 Waiver.Track incarcerated individuals ensuring department compliance with CAA and 115 Waiver requirements.Knowledge of, compliance with, and enforcement of Department policies and procedures.Coordinate and oversee all telehealth including virtual and telephonic meetings between incarcerated individuals and outside medical providers and partners.Assist in hiring, retention, and supervision of support staff.Obedient to moderately complex oral and written orders.Communicating effectively both orally and in writing.Deal calmly and effectively with stressful situations, including emergencies; and exercise good judgment.MINIMUM
