Physical Therapist – Full‑Time

• Responsible for the assessment of referred patients requiring rehabilitation services.
• Provide direct patient care to assess medical condition, functional capabilities, limitations and restrictions, and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to patient capacity and tolerance under physician direction.
• Set realistic and achievable goals for patients.
• Document and record patient condition and educate patients and families in appropriate physical therapy methods.
• Prepare evaluations, weekly progress notes, recertification, and discharge documentation.
• Collaborate with all disciplines to provide patient‑centered treatment and care.
• Perform all other duties assigned by the Director of Rehabilitation.
• Maintain confidentiality, HIPAA compliance, and federal residents’ rights.

• Graduate from an accredited physical therapy college or university.
• Hold a valid professional license and registration in the state of practice.
• Experience in long‑term or geriatric care is preferred but not required.
