Location: Greater Sudbury

Personal Plan Facilitator - Part-Time (20 hrs/week)

Rate of Pay: $29.48 - $33.30/hr

Bilingual in French and English is required.

Full G license is required.

Sudbury/Algoma

The Person al Plan Facilitator works with the person using services, their circle of support (including the Karis Disability Services’ team(s) that provides support) to learn about the person and use this information to facilitate the creation of a comprehensive Personal Plan. The plan will address the person’s needs, hopes and dreams with an eye toward building communities of belonging in accordance with the core Values of Karis Disability Services:
Valuing People, Fostering Belonging, Serving Others and Respecting Gifts.

  • Facilitation skills
  • Driver's License
  • Communication skills, written & oral
  • Interpersonal skills
  • French language proficiency is required (written and oral)
  • 5 years of experience in developmental services or other similar social services (e.g., children’s or seniors’ care)
Education
  • College diploma or university degree
Work Conditions
  • Frequent travel
  • Ability to work well under pressure both independently and as part of a team
  • Involves working with people who may be under significant personal stress
  • Intermittent periods of considerable mental demands
  • Hours may vary; flexibility, evening and weekend work may be required
  • Resourceful
  • Continual learner
