Job title:
Ambulatory Complex Care Manager at OSF Health Care
Salary/Rewards: Expected pay for this position is $29.30 - $34.47/hour. Actual pay will be determined by experience, skills and internal equity. This is a Salaried position.

Responsibilities- Promote the achievement of optimal clinical and resource outcomes by assessing, planning, implementing, coordinating, and monitoring interventions and developing a comprehensive plan of care for patients.
- Coordinate health care services to promote delivery of care at the appropriate level and collaborate with the Interdisciplinary Care Team to promote quality of care.
- Assess and address complex psychosocial and behavioral health issues and needs related to population management and transition plans.
- Work with the Care Management Division leadership team to ensure alignment with OSF Key Results and strategic goals.
- Education: REQUIRED QUALIFICATIONS:
Bachelor’s degree in Social Work (BSW) - Licensure/
Certifications:
Social Work; shall comply with state licensing requirements (Illinois and/or Michigan) or applicable jurisdiction. - Experience: 3 years of experience caring for patients
- Other Skills/Knowledge: Demonstrated broad knowledge of chronic illness and medical and community resources; working knowledge of a clinical EMR system and Microsoft Office; flexible work schedule including weekends, holidays, and on-call rotation; strong independent judgement and time management; excellent interpersonal skills; effective communication with patients/families across diverse backgrounds.
- Preferred Qualifications: Master’s degree in Social Work (MSW); experience with chronic, complex illnesses in adult and/or pediatric care management.
- MSW track:
Must start licensing application process by day 91 of hire; licensure within 1 year. - BSW track:
Identify clinical supervisor by day 91 of hire with a plan to complete supervision within 3 years; complete state licensure within 4 years from date of hire; detailed steps include annual application and supervision milestones.
Work 4 days from office and 1 from home after first 90 days.
