The Case Manager RN utilizes advanced nursing skills and knowledge to comprehensively coordinate care for a defined patient population in order to facilitate patients through the continuum of care.
Responsibilities- Advocates for the patient and family throughout the entire episode of care.
- Monitors patient’s progression, intervening as needed to ensure efficient patient progress.
- Coordinates all aspects of discharge planning and establishes a safe discharge plan.
Schedule:
Monday‑Friday 8:30 AM to 4:30 PM, 1 weekend day shift every 4 weeks, 1 major holiday, willingness to occasionally float to all 3 campuses.
- Required:
BSN; 2 years of medical surgical nursing experience. - Preferred: 1‑2 years acute care, homecare or population health RN case management experience; CCM certified.
$37.65 to $53.67 per hour, based on candidate experience and qualifications.
