Role and Conditions
  • Role: Complex Discharge Co-ordinator
  • Band: 6 £ 41,608 - £50,702 per annum
  • Allowance: Plus Distant Islands Allowance of £1,406 per annum
  • Hours: 37 hours per week
  • Status: Permanent

A new and exciting opportunity has arisen for an individual to play a key role in ensuring our patients enjoy as smooth and efficient a journey as possible following admission to the Western Isles Hospital. The successful applicant will work collaboratively and closely with colleagues across health and social care partnerships to both anticipate and plan for the patient journey from admission to discharge.

They will act as the main point of contact for patients, their families, and colleagues in tertiary centres seeking to repatriate patients back to the Western Isles hospital for ongoing care.

Responsibilities
  • Play a key role in ensuring patients have a smooth and efficient journey from admission to discharge.
  • Work collaboratively with colleagues across health and social care partnerships to anticipate and plan patient journeys.
  • Act as the main point of contact for patients, their families, and colleagues in tertiary centres seeking repatriation to the Western Isles hospital for ongoing care.
Qualifications and Requirements
  • Excellent communication skills.
  • Ability to problem solve and coordinate patient care within the acute setting in conjunction with wider clinical teams.
  • Ability to work within the context of the wider clinical teams and systems.
