Overview
We are seeking a Registered Dietitian to join our team and provide nutritional assessment, counseling, and education to support the health and well-being of residents at the Long Island State Veteran's Home (LISVH). This role is responsible for developing individualized nutrition care plans, monitoring dietary compliance, and collaborating with interdisciplinary teams to ensure optimal clinical outcomes and regulatory compliance.

- Master’s degree in Dietetics, Nutrition, or related field.
- Registered Dietitian (RD/RDN) credential from the Commission on Dietetic Registration (CDR).
- Comprehensive knowledge of Medical Nutritional Therapy and documentation standards for long-term care.
- Minimum of 1 year experience in long-term care.
- Strong interpersonal and communication skills; ability to collaborate effectively within a multidisciplinary team.
- Proficiency in English (speaking, reading, and writing).
- Ability to meet background check standards.
- Active membership in the Academy of Nutrition and Dietetics.
- Member of Long Island Dietetic Association.
- Conduct comprehensive nutritional assessments upon admission and at regular intervals.
- Develop individualized dietary plans based on medical history, assessment findings, and resident preferences.
- Monitor and evaluate residents’ nutritional status; adjust plans as needed.
- Provide ongoing nutrition education and counseling to residents, families, and caregivers.
- Collaborate with nursing and other healthcare professionals to optimize care plans.
- Attend care plan meetings and complete required documentation in accordance with facility and Department of Health standards.
- Ensure compliance with federal, state, and facility-specific guidelines.
- Oversee therapeutic diets and maintain accurate documentation.
- Mentor dietetic interns as assigned.
- Stay current with advancements in dietetics and incorporate evidence-based practices.
- Perform other duties as assigned by the Director of Nutritional Services.
