Job Description - LPN - Transitional Care Unit - Full Time Days 11am-7pm (2504698)
Duties- Collect data through observation and communicate information to assist the RN and/or provider in patient assessment and care planning.
- Participate as a member of the health care team and accept delegation from the RN/assigned supervisor and provider in meeting the needs of the patient/family.
- Deliver care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care.
- Possess excellent communication skills; mentor and teach; participate on committees or projects.
- Plan and manage patient care according to each patient’s needs.
- Interview patients and record medical history and physical condition.
- Obtain patient vital signs, including pulse, blood pressure, temperature, and respiration.
- Provide routine care for patients and accompany patients to their rooms.
- Assist physicians and registered nurses.
- Communicate with patients and their families.
- Perform standard diagnostic procedures including EKG, phlebotomy, venipuncture, PFT, and Doppler.
- Prepare specimens for lab courier and provide wound care.
- Prepare and administer injections and/or enemas.
- Assist with patient comfort.
Working conditions may involve exposure to infectious disease, blood, body fluids and/or tissue, and other elements such as accidents, injuries and illness. May be exposed to varying unpredictable, emergency and crisis situations. Occasionally subjected to irregular hours and pressure. This position is classified by OSHA as Category 1.
Qualifications- Required:
Must possess or be eligible to obtain a license to practice as a Licensed Practical Nurse in New York State. - Preferred:
Acute care, phlebotomy, EKG, and/or IV experience. - As part of onboarding, a copy of your current Basic Life Support (BLS) certification is required. If BLS expires within 90 days of start, you will attend a BLS training session during nursing orientation.
- Special note:
Acute care is defined as medical services for short-term, intense illnesses requiring vigilant monitoring and quick recognition of potential problems.

CompensationAnticipated Pay Range:
The salary range is $45,081 per year. Additional compensation may include location pay, shift differentials, and other stated benefits. Total compensation includes leave, health plans, and a state pension.
