We are seeking a compassionate and detail-oriented Registered Nurse (RN) to perform in-home patient assessments for our growing home care program. This role focuses on evaluating patient needs, ensuring regulatory compliance, and helping families receive the right level of care.

The RN Home Care Assessment Nurse conducts comprehensive in-home evaluations for new and existing clients to determine eligibility, level of care, safety risks, and service needs. This nurse plays a critical role in ensuring high-quality, compliant, and patient-centered care.

• Conduct initial and follow-up in-home nursing assessments
• Evaluate physical, cognitive, and psychosocial status
• Identify safety risks, fall risks, and home environment concerns
• Verify diagnoses, medications, and care needs
• Determine level of care and service eligibility
• Document assessments clearly and accurately in the medical record
• Communicate findings to care coordinators and clinical leadership
• Support regulatory compliance with CMS, state, and agency policies

• Active Registered Nurse (RN) license in [State]
• Minimum 1 year of clinical experience (home health, hospice, or case management preferred)
• Strong assessment and documentation skills
• Comfortable working independently in patient homes
