Overview
The role involves working with and supporting renal transplant recipients in the outpatient setting throughout their transplant journey, including patient assessment up to transplant list placement and post‑transplant care.
ResponsibilitiesWork with patients to prepare them for the transplant list, provide ongoing care post‑transplant, and collaborate with the multidisciplinary team to ensure optimal patient outcomes.
