About the Role

This Band 7 post is part of our Crisis Resolution and Home Treatment Team (CRHTT). The post holder will provide advanced clinical expertise, leadership and autonomous decision‑making within the multi‑disciplinary team.

Responsibilities
  • Provide senior clinical leadership and autonomous decision‑making in the management of acutely unwell service users in community settings.
  • Undertake comprehensive mental health assessments, including formulation and complex risk management.
  • Lead on admission avoidance and early discharge to reduce inpatient pressures.
  • Prescribe and manage treatment plans within scope of practice; collaborate with medical colleagues where required.
  • Liaise with inpatient wards, community services and partner agencies to ensure continuity of care.
  • Contribute to service development, quality improvement and clinical governance activities.
Main Duties

Support the team to deliver a high‑quality mental health service for individuals presenting in crisis with acute needs. Provide clinical leadership, training, specialist assessment and time‑limited interventions. Act as a source of expertise and point of contact within the team, demonstrating high levels of decision‑making and clinical judgement.

Person Specification Qualifications
  • Registered Nurse (Mental Health) with current NMC registration.
  • Non‑medical prescribing qualification (preferred).
  • Evidence of continuing personal learning and development.
Experience
  • Experience in an older‑age speciality or equivalent.
  • Demonstrated leadership and change management.
  • Experience working across organisational boundaries to develop and deliver services.
  • Evidence of significant contribution to service developments and education of multi‑professional groups.
  • Experience of applying research in practice and research experience (a).
