Baddeley Green Surgery is looking for an experienced Primary Care Nurse to be responsible for the delivery of practice nursing services, workingas part of the practices multidisciplinary team who provide care within theirscope of practice to the patient population. The Practice Nurse will be responsible for a number of clinical areas such as Infection Control, Immunisation Lead, Health Promotion, Chronic Disease Management, Health Prevention, Well Womanand Well Man clinics, as well as supporting the management team in thereviewing of clinical policy and procedure.
Mainduties of the job
Develop, implement and embed health promotionand well-being programmes Implement and evaluate individual treatment plans for chronic disease patients Identify, manage and support patients at riskof developing long-term conditions, preventing adverse effects on patients health Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE, NSF and otherlegislative frameworks and guidelines Provide wound care (ulcer/Doppler etc.) to patients Provide travel vaccination and advisory services Request pathology services as necessary Process pathology results as required Urinalysis Provide chronic disease management, delivering patient care as necessary, referring patients to secondary/specialist care as required INR Star

Baddeley Green Surgery consists of a Senior Partner, two salaried GP's, one long term locum GP, ANP, Practice Nurse and two HCSWs. The practice has a growing population with approximately 5,900 patients at present.
The practice has a compliment of Managerial, Administrative and Secretarial staff in order to provide an efficient and effective service to all our patients.
Job responsibilitiesDevelop, implement and embed health promotionand well-being programmes

Assist GPs with minor surgery when required
Prioritise health issues and intervene appropriately
Support the team in dealing with clinical emergencies
Recognise, assess and refer patientspresenting with mental health needs
Implement vaccination programmes for adultsand children
Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
Liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients etc.)
Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
Support the clinical team with all safeguarding matters, in accordance with local and national policies
Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
Deliver opportunistic health promotion where appropriate
Act as the audit lead, effectively utilising the audit cycle
Monitor and ensure the safe storage, rotationand disposal of vaccines or medication
Support junior members of the nursing team,providing guidance when necessary
Participate in local initiatives to enhanceservice delivery and patient care
Support and participate in shared learningwithin the practice
Continually review clinical practices,responding to national policies and initiatives where appropriate
Participate in thereview of significant and near-miss events applying a structured and methodical approach with learning outcomes to ensure non-recurrence
Person Specification Qualifications- NMC Registered Nurse
- Post Registration Qualifications, Diabetes Management, Asthma
- Postgraduate diploma or degree (Primary Care)
- Experience of working in a Primary Care environment
- Experience of working as a Practice Nurse
- Experience of Chronic Disease Management
- Experience of leading a team
- Wound care/removal of sutures and staples
- ECGs
- Venepuncture
- New patient health checks/Urinalysis/BP/Height/Weight
- Chaperone procedure
- Requesting pathology tests and processing the results, advising patients accordingly
