An exciting opportunity has arisen to join the 0-19 Stoke Integrated Health Visitor and School Nurse Service, delivering public health services to children and families across Stoke on Trent.
The role is a 22.5‑hour a week Band 6 Public Health Practitioner (HV/SN) based in the 0-19 Stoke Hub at Smallthorne Health Centre.
Key Responsibilities- Provide compassionate, warm and approachable care, actively listening to children, young people and families.
- Lead by example and encourage effective skill‑mix team working to develop colleagues and achieve the best outcomes for constituents across Stoke on Trent.
- Deliver professional and personal development opportunities, including the Transfer of Fields (TOF) Training to develop knowledge of the 0‑5 and 5‑19 elements of the service; commence the relevant course within the first 12 months of employment.
- Be competent within the expectations of the role and work within the agreed scope of practice.
- Demonstrate the Values of Midlands Partnership Foundation in all activities.
- Act as a Specialist Community Public Health Practitioner with a focus on the public health 0‑19 agenda.
- Manage a defined population of service users in the community and other settings across the Partnership Trust.
- Assess children and young people’s care/support needs and develop, implement and evaluate care/support plans.
- Hold supervisory, training and leadership responsibilities, contributing to team performance, service development and improvement.
- Demonstrate initiative and creativity in finding solutions to challenges.
- Work with the team leader, undertaking delegated tasks within the team.
- Deliver commissioned aspects of the Healthy Child Programme, ensuring key objectives and outcomes are met.

- Qualified SCPHN – Health Visitor or School Nurse.
