Our mission revolves around providing essential services such as education, referral to substance use disorder (SUD) programs, support for homeless individuals, cultural education, and drug and alcohol prevention. - Resource Navigation:
Conduct comprehensive assessments of individuals’ needs and circumstances and provide personalized assistance and guidance in accessing relevant resources and services. - Referral Coordination:
Collaborate with community partners, service providers, and agencies to facilitate referrals for clients to appropriate programs and resources, ensuring continuity of care and support. - Case Management:
Maintain detailed records of client interactions, referrals made, and follow-up activities, and provide ongoing support and advocacy to address barriers and challenges. - Information and
Education:
Provide information and education to clients about available resources, eligibility criteria, application processes, and rights and responsibilities, empowering them to make informed decisions. - Crisis Intervention:
Provide crisis intervention and support to individuals experiencing urgent or emergency situations, connecting them with appropriate services and resources. - Cultural
Competency:
Demonstrate cultural humility and sensitivity in all interactions, respecting and honoring the traditions, values, and perspectives of the Native American community. - Collaboration:
Collaborate closely with other members of the Native C.O.R.E. team, including outreach coordinators, program staff, and community partners, to ensure coordinated and effective service delivery. - Documentation and Reporting:
Maintain accurate and up-to-date documentation of client interactions, services provided, and outcomes achieved, and prepare regular reports as required.
- Bachelor’s degree in social work, psychology, human services, or a related field.
- Minimum of 1 year of experience in case management, resource coordination, or social services, preferably in a nonprofit or community-based organization.
- Knowledge of and experience working with Native American communities, with a demonstrated commitment to cultural competency and understanding.
- Strong interpersonal and communication skills, with the ability to build rapport, establish trust, and communicate effectively with diverse populations.
- Ability to work independently and as part of a team, with a proactive and solution-focused approach to service delivery.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple deadlines.
- Proficiency in Microsoft Office Suite and experience with database management preferred.
- Valid driver’s license and willingness to travel locally as needed.
