LPN LVN Full Time PM Shift — Creekside Center

Join us as a Licensed Vocational Nurse/Licensed Practical Nurse (LVN/LPN) to provide high-quality care in a compassionate, family‑oriented environment.

Salary Range: $35.00–$39.00 per hour

• Admitting, transferring, and discharging residents.
• Conducting daily rounds to observe the resident’s physical and emotional status.
• Medication administration.
• Monitoring resident care provided by unlicensed staff.
• Maintaining the confidentiality of all resident care information.
• Promoting a culture of safety to ensure a healthy practice and living environment.

• LVN/LPN with current licensure to practice in the state.
• Strong communication and organizational skills.
• Basic knowledge of computer use or willingness to learn.

• Experience in a Skilled Nursing Facility (SNF) environment.
• Nursing experience providing direct care to multiple residents.
• Behavioral care / memory care experience.
• Experience with Point Click Care  (PCC).
