Speech Language Pathologist (SLP) – Home Health, Stanislaus County

Position summary:

As a Home Health Speech Language Pathologist, you will make home visits to assess and treat speech, language, social communication, cognitive communication, and swallowing disorders. You will provide detailed notes and documentation regarding home therapy visits and patient progress.

• Knowledge of the principles and practices of speech pathology and provide speech therapy services in accordance with the state practice act
• Perform an initial assessment and evaluation of the patient and establish a plan of care, including scheduling subsequent visits determined by the SLP to be clinically appropriate
• Coordinate care through care conferencing with other healthcare team members
• Observe, record, and report to the Primary Care Physician the patient’s treatment, response, and progress including significant changes
• Obtain orders to revise the plan of care as clinically appropriate
• Perform re-evaluations of the patient’s progress at least every 30 days and update the plan of care as necessary
• Complete comprehensive assessments and OASIS data as necessary
• Educate patients and caregivers about the patient's impairments and provide supporting materials to foster independence
• Other duties as assigned

• Master's degree in Speech Language Pathology
• Active Speech Language Pathology license (CCC)
• Ability to practice speech therapy unencumbered within state and federal guidelines
• At least one year of experience working as a Speech Language Pathologist (no CFYs)
• Home Health and Home Care documentation experience in Home Base
