Advanced Practice Clinician (NP/PA) – Home Visits in San Joaquin County, CA

The Advanced Practice Clinician in the Care Anywhere (CAW) program provides care and support to our Alignment Healthcare patients by delivering medical and social support in the comfort of their own homes. This program is offered at no cost to eligible patients to prevent unnecessary hospitalizations and health complications.
Responsibilities- Conduct in‑home assessments on Alignment members, medication review, and health screenings
- Provide patient education by matching desired care with best practices
- Identify diagnoses for active medical and care management
- Partner and communicate with Regional Medical Officer and team members to develop appropriate care plans for members
- Coordinate care with multiple stakeholders, including PCPs, specialists, and ancillary providers
- Lead broader clinical team (nurses, health coaches, social workers, and care coordinators)
Job Requirements
- Required
:

:
One (1) year of prior clinical or home‑care experience; previous EMR experience; experience caring for older adults (geriatric patients)
- Required
:
Master’s degree from an accredited NP or PA program
- Effective communication – positive, professional, and effective with others
- Problem‑solving, organizational, time‑management, and ability to work in a fast‑paced environment
- Required – Active Nurse Practitioner or Physician Assistant license
- Active RN license and furnishing number (must upon hire for NP only)
- Board certification (NP or PA)
- NPI Number, DEA, Valid BLS
- Valid driver’s license and current automobile insurance
- Regularly required to talk, hear, stand, walk, sit, use hands, and reach with arms
- Frequently lifts and/or moves up to 20 pounds
- Vision: close vision and ability to adjust focus when needed
