Physical Therapist / PACE Program
Stockton, CA
$100,000 – $130,000 Base + Bonus + Equity
The OpportunityWe are a mission-driven healthcare organization dedicated to serving seniors with a holistic, human-centered approach. We are looking for a Physical Therapist to join our Interdisciplinary Team (IDT).

- Holistic Care Model:You will act as a core member of an Interdisciplinary Team (IDT), collaborating daily with doctors, social workers, and other specialists to care for the whole person.
- Meaningful Relationships:Move beyond short-term rehab stints. You will form long-term relationships with participants, helping them age with dignity in their own communities.
- Assessments:Conduct Physical Therapy assessments in our center (and occasionally in-home) to define care needs, preferences, and functional goals.
- Care Planning:Participate actively in IDT meetings to assist in the development and adjustment of participant care plans.
- Interventions:Deliver and document interventions utilizing active/passive exercise, muscle re-education, gait training, and functional training for ADLs.
- Therapeutic Modalities:Utilize treatments including soft tissue mobilization, thermal agents, ultrasound, hydrotherapy, and electrical stimulation as indicated.
- Supervision:Supervise Therapy Assistants (PTAs), providing the mentorship needed to implement Functional Maintenance Programs (FMP).
- Education:Train staff, participants, and family caregivers on the safe use of therapeutic equipment and home safety.
- Education:Bachelor’s degree in Physical Therapy required;
Master’s or Doctorate (DPT) preferred. - Licensure:Valid California Physical Therapy License (or proof of eligibility).
- Experience:
- 2+ years of clinical experience preferred (Outpatient, Acute, Inpatient, or Skilled Nursing backgrounds welcome).
- Experience with a frail or elderly population is highly preferred.
- Certifications:Current CPR certification (preferred).
