The Welbe Health PACE program helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Home Care Supervisor plays a critical role in monitoring our team and ensuring the needs of our participants are met with the highest level of care.
Key Responsibilities- Oversee the Home Care Assistant team providing activities of daily living to participants in their homes.
- Interview, hire, onboard, train, and provide feedback to Home Care Assistants ensuring alignment with Welbe Health standards.
- Provide feedback to agencies/contracted vendors on caregiver performance and participant interactions.
- Maintain records of compliance with certifications for certified Home Care Assistants and ensure credentials are current.
- Regularly audit accuracy and timeliness of Home Care Assistant documentation and provide training on areas needing improvement.
- Collaborate with the Home Health Manager and Rehabilitation department to ensure Home Care Assistants receive training for home exercise programs as indicated in care plans.
- High School Diploma or Equivalency;
Bachelor’s Degree preferred. - Minimum one year of experience overseeing a team of Home Care Assistants for a frail or elderly population.
- Two years of supervisory experience with a demonstrated ability to mentor and develop team members.

Salary base range: $74,612 – $98,488 per year, plus bonus and equity.
