Physical Therapist – Stockton

Compassionate Care Home Health is seeking a dedicated Physical Therapist to serve patients across the Central Valley. We are looking for a clinician who can provide high‑quality therapy, collaborate with a multidisciplinary team, and support patient recovery in a home‑health setting.

• Provide physical therapy services according to a written physician’s plan of care and the state Physical Therapy Practice Act.
• Assess and evaluate therapeutic/rehabilitative/functional status, and assist in the development of the total plan of care.
• Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances.
• Determine priority needs for physical therapy and report progress to the physician.
• Initiate physical therapy programs, instruct personnel and caregivers, and participate in case conferences.
• Train patients in the use of prosthetic devices and identify needs for other home‑health services.
• Prepare and submit clinical and progress summaries, participate in discharge planning, and provide in‑service education to nursing personnel.
• Supervise physical therapy assistants and, when therapy is the only skilled service, supervise home‑health aide care every two weeks.

• Master’s or doctoral degree in physical therapy from an accredited program.
• Licensed to practice as a physical therapist in California (if applicable).
• One year of community/home health experience preferred.
• Strong verbal and written communication and organization skills.
• Current CPR certification.
• Valid driver’s license, reliable vehicle, and vehicle insurance in accordance with company requirements.
