Your Role

Edge of Care is an innovative, dynamic and creative service, that has a clear aim to safely prevent and reduce the number of children and young people from entering the care system in Stockport. Working with the Edge of Care services as a Senior Practitioner you will be part of a wider resourceful team. Sharing a passion for providing young people with the best possible outcomes in life. You will be part of a service that is in the early stages of re-design.

You will make the most of your social work and specialist experience by holding, directing and supporting a team of Intensive Support workers and taking a lead in the development of their practice. Within the role you will be having an oversight of all of the cases that are referred through Social Care.

The Edge of Care Service keeps Children and Young People at the Heart of all their work, they listen, respond and value the families and communities.

This is an exciting opportunity to have a “Key Role” in combining direct practice alongside service and professional development.

The Advanced practitioner will assist the Team Manager to deliver effective coordinated evidence-based support and interventions to young people and their families/ carers to;

  • Support and maintain the child in their family home, where without an intensive package of support, it is reasonable to expect a family breakdown.
  • Support a child in their placement, where without an intensive package of support, it is reasonable to expect a placement breakdown.
  • Enable a child who is already looked after to return home with the support of an edge of care intervention.

To work as part of a multi-agency team offering time-limited intensive interventions and parenting support to improve the outcomes of young people and their families, where universal and single agency targeted services have failed to achieve sustainable changes.

Building effective relationships with young people, their families and key partners (especially Health, Schools, Children’s Centres and Housing) to ensure a fully integrated and holistic approach to ultimately provide a seamless service that empowers service users.

To provide flexible intensive support services for young people and their families in crisis where immediate support is required, planned work and maintenance support to meet children and their families holistic needs at the earliest point and at the times they require the support. The team operating hours will be between 8am and 8pm with the facility up to 10pm and sleep in seven days per week around the needs of the children and families which may change as their needs change.

The hours will include working day time, out of hours including bank holidays, with at least 10% of hours working at weekends.
