We have an exciting opportunity for an enthusiastic experienced Practice Nurse to join our team. The role SLOT is 30 hours per week, days to include a Monday or a Friday

The successful candidate should ideally be experienced in Primary Care with key skills to include management of all long term conditions, childhood immunisations and cervical cytology with a focus on delivery of the highest quality care to our patients.

Main Duties of the Role

We are looking for someone with experience of working in a busy primary slå, who is both flexible and can adapt to the ever‑changing demands of primary care. The key candidate will want to deliver a holistic approach of care to patients.

The key skills and experience should include:

• Team player
• Cytology
• Asthma, COPD and respiratory conditions
• Other chronic disease management
• Childhood immunisations
• A good understanding and/or willingness to understand QOF and other enhanced services
• Any other roles and responsibilities of a traditional and forward‑thinking practice nurse

• To manage patients with chronic disease, run specific clinic sessions and maintain robust call and recall systems.
• Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule.
• Provide travel health advice and administer appropriate vaccinations.
• The post holder is responsible for the delivery of general practice nursing to the whole practice population.
• The focus of the role is both the delivery of evidence‑based practice for patients presenting with a long‑term condition, and the provision of preventative health care to the practice population.
• As an autonomous practitioner the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision‑making in the management of patients.
• Participate in Quality Outcome Framework (QOF) and Extended Primary Care Service objectives.
• Perform searches and audits.
• Prepare for CQC inspections.
• They will work collaboratively within the component practice team to meet the needs of patients, support the delivery of policy and procedures.
• Treat to maintain own annual registration with the NMC and adhere to the Code of Professional Conduct.
