Job Summary
Stockport Foundation Trust is looking for a dynamic, experienced Leader to join the Leadership Team within the Directorate of Out of Hospital, Urgent Community Services. The role is pivotal for the Hospital @ Home Service (Virtual Ward) and the newly created Digital Hub, working closely with leads in Urgent Community Response and Advanced Practitioner service.
Key Responsibilities- Ensure efficient and effective running of services.
- Supervise delivery of pathway services, collaborating with Clinical Leads, Team Leads, and staff.
- Develop and implement operational management systems, process pathways, SOPs, protocols, and strategies.
- Lead and challenge the team to deliver evidence‑based, effective and efficient care.
- Use a LEAN methodology to maximise efficiency.
- Lead stakeholder engagement on operational objectives and evaluation.
Education & Qualifications (Essential)
- Registered Nurse or equivalent professional qualification.
- Management/leadership qualification or equivalent experience.
- Postgraduate study at an advanced level or equivalent experience.
- First‑level degree or equivalent experience.
Desirable
- Coaching qualification or experience.
Knowledge (Essential)
- Coaching qualification or experience.
- Understanding of clinical risk, incident management, patient safety, and quality improvement initiatives.
- Understanding of research and evidence‑based practice.
Experience (Essential)
- Senior clinical or managerial role experience of 2+ years.
- Significant practical contribution to service development.
- Experience managing complex patients and advanced communication in stressful situations.
- Continuous professional development and practical application.
- Experience in a senior Community role (Band 7 or above).
Skills & Abilities (Essential)
- Confidence to assess and manage complex patients.
- Documented advanced clinical skills and competencies.
- IT literacy.
- Confidence in discussions with senior colleagues and partner organisations.
- Commitment to multidisciplinary teamwork.
- Patient‑focused with a commitment to high‑quality care.
- Current driving licence; car ownership with insurance for business use.
- Ability to remain professional, calm and polite under pressure.
- Flexible and adaptable approach to work.
