Stockport Foundation Trust is currently offering an exciting opportunity for an Advanced Clinical Practitioner, fixed term contract to join their dynamic service.
The post will be full time and be based in the community setting.

Main duties of the jobThe post holder will be responsible for providing advanced clinical care and expertise in care delivery including judgement, discretion and decision making to patients as per individual scope of practice and through the advanced clinical competency framework.

Work autonomously as an ACP within the specialty, managing a caseload of patients and clinical priorities according to agreed protocols and working practices.
Deliver individualised patient care, with direct responsibility for assessment, examination, investigation/ diagnostic procedures, and diagnosis of patients with undifferentiated and undiagnosed presentations.
Appropriately treat patients, resulting in the safe management and appropriate onward referral or discharge of patients at the end of the care episode.
Receive and triage referrals via a variety of sources, including direct patient referral.
Participate in ward rounds, patient reviews, and multidisciplinary team meetings.
Utilise scope of advanced practice to undertake Non-Medical Prescribing role and advise on medicine management issues associated with the patient group, working within Trust policy for Medicines Management.
