- Conduct Regulation 24 assessments
, viability assessments
, and full connected carers assessments in line with statutory requirements. - Provide practical and emotional support to connected carers, ensuring they are fully prepared to meet children’s needs.
- Develop and implement sustainable care plans in partnership with families and other agencies.
- Liaise effectively with schools, health professionals, and partner organisations to deliver cohesive care solutions.
- Deliver training and support sessions to connected carers, enhancing their understanding and confidence in their roles.
- Maintain accurate records, reports, and documentation in accordance with service and legal standards.
You will be a qualified Social Worker registered with Social Work England
, bringing at least two years of experience within a fostering or connected carers team. You have proven ability to complete comprehensive assessments and a strong understanding of fostering regulations, child safeguarding, and permanency planning.
