Town Break Dementia Support is recruiting a Dementia Support Coordinator to play a central role in delivering warm, person-led support to people living with dementia and their unpaid carers across Forth Valley.

About the RoleAs Dementia Support Coordinator, you will:
- Act as a named coordinator for people accessing Town Break services
- Coordinate and deliver community‑based support, including Town Break at Home and group activities
- Be a key point of contact for people living with dementia and their unpaid carers
- Work closely with partners across health, social care and the third sector
- Contribute to service development, quality improvement and partnership working
Town Break at Home runs Monday to Friday and offers personalised one‑to‑one support, either at home or out in the community. Support is shaped around each person’s strengths, interests and goals.
Key Responsibilities 1. Support & Coordination- Hold strengths‑based conversations to identify goals and outcomes
- Coordinate agreed support and review needs regularly
- Provide information, guidance and signposting to relevant services
- Work collaboratively with families, unpaid carers and partners
- Participate in the duty rota
- Coordinate and facilitate inclusive, engaging activities and groups
- Support one‑to‑one and group delivery, including cover when needed
- Source venues, complete risk assessments and manage attendance records
- Gather feedback and use it to shape service development
- Support the recruitment, supervision and ongoing support of volunteers
- Ensure volunteers feel confident, supported and aligned with our values
- Work in line with safeguarding, data protection and organisational policies
- Promote equality, accessibility and inclusion in everything you do
- Contribute to team meetings, reflective practice and service planning
- Act as a positive ambassador for Town Break
- An understanding of dementia and its impact on individuals and families
- A commitment to person‑led, strengths‑based support
- Strong communication and relationship‑building skills
- Ability to manage a varied workload and work independently
- Full UK driving licence and access to a car
- Experience working or volunteering with people living with dementia
- Knowledge of the voluntary, health or social care sectors
- Confidence using Microsoft 365 and basic databases
- Lived experience of dementia, personally or through caring roles
