This role offers a flexible 40hr weekday schedule
, strong clinical support, and the opportunity to make a real impact in patients recovery journeys—right where they need it most.

• Flexible Scheduling: Choose between five 8-hour shifts or four 10-hour shifts
• Work–Life Balance: Monday–Friday schedule;
  weekends are very rare
• Diverse Clinical Exposure: Home health with acute care, inpatient med/surg, and rehab focus

• Provide skilled physical therapy services to home health patients based on individualized plans of care

• Evaluate patients, develop treatment plans, and modify care based on progress and functional goals

• Deliver therapeutic exercises, gait training, wound care, and functional mobility interventions

• Educate patients and families on home exercise programs, safety, and use of equipment

• Collaborate closely with physicians, case managers, and the multidisciplinary care team

• Accurately document evaluations, progress notes, and discharge summaries in a timely manner

• Ensure compliance with Medicare, safety, and home health regulatory requirements

• Support continuity of care and participate in team meetings as needed

• Graduate of an accredited Physical Therapy program (Bachelors degree or higher)

• Active Oklahoma Physical Therapy license

• CPR certification (or ability to obtain within 30 days of hire)

• Strong communication and documentation skills

• Valid Oklahoma drivers license, reliable transportation, and auto insurance

• Ability to work independently in home settings and manage a varied caseload

• Comfort working with pediatric, adult, and geriatric populations

• Ability to lift, transfer, and assist patients (up to or exceeding 100 lbs. as needed)

• Ability to stand, walk, bend, and stoop throughout the workday

• Capacity to work in home environments and manage occasionally stressful situations
