North Shore at Home Full‑time Physical Therapist Sheboygan and Stevens Point Wages up to $50/hr, based on experience
This job description outlines the essential functions of the Physical Therapist position. It provides employees, the Administration, Personnel Department, applicants, and other agencies with a clear understanding of the job, where it fits in the organization, and the skill and work requirements in relation to other jobs.
Qualifications- Current license in the State of Wisconsin/Illinois.
- Must provide a health statement signed by a physician.
- Must show evidence of a current negative tuberculin skin test or x‑ray; annual testing required unless prior positive TB test documented.
- Must be free of physical/medical conditions that would limit or restrict ability to perform the job functions.
- Provide physical therapy services prescribed by the physician that can be safely provided in the home. Complete initial evaluation and admission visit within 48‑72 hours of referral for patients being admitted for physical therapy and develop a plan of care.
- Assist the physician in evaluating patients by applying diagnostic and prognostic tests for muscle, nerve, joint and functional abilities.
- Supervise physical therapy assistants.
- Treat patients to relieve pain, develop or restore function, and maintain maximum performance using physical means such as exercise, massage, heat, water, etc.
- Arrange the provision of services listed above on an outpatient basis which cannot be given in the patient’s home.
- Observe, record, and report to the physician and other agency staff the patient’s reaction to treatment and other changes in the patient’s condition or plan of care.
- Instruct patients and/or caregivers in the care and proper use of equipment and devices such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic and orthotic devices.
- Instruct family members and other health team personnel, including, when appropriate, home health aides, in certain phases of physical therapy so they can work with the patient.
- Develop a patient care plan and measure progress or lack of progress in goal achievement.
- Responsible for the physical therapy plan of treatment, daily physical therapy summaries, and discharge summaries.
- Participate in orientation of new employees and in‑service programs related to physical therapy programs.
- Participate in case conferences and meetings as needed.
- Maintain required hours of in‑service training per calendar year and attend mandatory in‑services.
