Full-time Physical Therapist
North Shore at Home – Sheboygan and Stevens Point
Wages up to $50/hr, based on experience
This position requires a Physical Therapist with a current license in the State of Wisconsin/Illinois.
Health RequirementsMay provide a health statement signed by a physician and must show evidence of a current negative tuberculin skin test or x-ray. Thereafter an annual tuberculin skin test is required, unless the employee has documentation of a previous positive T.B. test. Employee must be free of physical/medical conditions which would limit or restrict their ability to perform the job functions listed below.
EssentialFunctions
- Provides physical therapy services prescribed by the physician that can be safely provided in the home. Completes initial evaluation and admission visit within 48-72 hours of referral for patients being admitted for physical therapy and develops a plan of care.
- Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities test.
- Supervises physical therapy assistants.
- Treats patients to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, etc.
- Arranges the provision of services listed above on an outpatient basis which cannot be given in the patient's home.
- Observes, records, and reports to the physician and other agency staff the patient's reaction to treatment and other changes in the patient's condition or plan of care.
- Instructs patients and/or caregivers in the care and proper use of equipment and devices such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic and orthotic devices.
- Instructs family members and other health team personnel, including, when appropriate, home health aides, in certain phases of physical therapy so they can work with the patient.
- Develops a patient care plan and measures progress or lack of progress in goal achievement.
- Responsible for the physical therapy plan of treatment, daily physical therapy summaries, and discharge summaries.
- Participates in orientation of new employees and in-service programs related to physical therapy programs.
- Participates in case conferences and meetings as needed.
- Maintains required hours of In-service training per calendar year, and attends mandatory In-services.
