Overview

The Loudoun County Department of Mental Health, Substance Abuse and Developmental Services is seeking a talented and motivated individual to serve as a Support Coordinator in the Support Coordination program. The Department of Mental Health, Substance Abuse and Developmental Services seeks a Support Coordinator with the Developmental Services Division. This position is responsible for assessing, coordinating, monitoring, and linking service delivery for children and/or adults with intellectual and developmental disabilities and their families. Experience with the HCBS waivers, CCC Plus Waiver, and Medicaid is preferred. Additionally, knowledge of community‑based resources including integrated housing options and employment first principles is desired.

Responsibilities
  • Assess eligibility and needs of the individuals, including risk;
  • Interview individuals and their families;
  • Facilitate team meetings and coordinate service delivery among providers;
  • Develop and document person‑centered plans using a strengths‑based approach;
  • Assure quality service provision and maintain compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention;
  • Demonstrate effective oral and written communication skills, strong organizational and time‑management skills, and proficiency in utilizing electronic health records and other software programs for documenting service delivery.
Requirements
  • Bachelor's degree (Master's degree preferred) in Human Services, Child Development, Early Childhood Special Education, Social Work, Counseling, Psychology, or related field; three (3) years of related experience in assigned program area preferably as a case management, service coordinator, or multi‑disciplinary team facilitator with children, youth, adults, and families; or equivalent combination of education and experience.
Preferred Qualifications
  • Program experience with individuals with developmental disabilities and the ability to communicate in Spanish using all modes of communication.
