Overview

Pomeroy Living is seeking a compassionate and experienced Social Worker to join our team. As a key member of our interdisciplinary care team, you will play a vital role in providing psychosocial support and advocacy to our residents.

Responsibilities
  • Conduct comprehensive psychosocial assessments of residents, including identifying social, emotional, and spiritual needs.
  • Develop and implement individualized care plans that address resident needs and support their overall well-being.
  • Provide counseling and support to residents and their families on a variety of issues, such as grief and loss, adjustment to care, and end-of-life care.
  • Advocate for resident rights and ensure their quality of life.
  • Assist with discharge planning and coordinate community resources.
  • Collaborate with the interdisciplinary care team, including physicians, nurses, therapists, and other staff.
  • Maintain accurate and timely documentation in accordance with regulatory requirements.
  • Participate in facility-wide quality improvement initiatives.
  • Conduct resident and family meetings as needed.
  • Provide support and guidance to staff on psychosocial issues.
Qualifications
  • Minimum of Bachelor’s Degree in Social Work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology, Master’s Degree preferred. One year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation.
  • Current Social Work license in the state of Michigan preferred
  • Excellent communication, interpersonal, and documentation skills.
  • Strong assessment, problem-solving, and critical thinking skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Passion for working with older adults and a commitment to providing high-quality care.
